
During the course of the school year, your child may need to take medication while at school. Over-the-counter and prescription medicines require a completed “Authorization for Administration of Medication at School” form on file in the nurse’s office in order for your child to be given medicines at school. Please note that this form includes the physician’s signature as well as a parent/guardian signature. Washington State law prohibits teachers from administering any medicine; therefore, it must be dispensed from the nurse’s office, but medication must accompany this form. If a student is on a field trip, the requirements are the same for administration, although teachers will administer medications instead of office staff.
No medication, over-the-counter or prescription should be sent in the student’s lunch, backpack or should be in a student’s pockets. This includes, but is not limited to, aspirin, Tylenol, cough remedies/drops, decongestants, lip balm/chap stick, and antihistamines. It is very handy to keep a copy of this form in your car glove compartment in case you visit your child’s physician and medication is recommended or prescribed.
